Understanding the HPO axis (in plain language)
Your fertility runs on a three-way conversation between your brain, pituitary gland, and ovaries. Here's how it works β and why it matters.
If you've ever been told "your hormones are off" without anyone explaining which hormones, or why, this post is for you. Almost every modern fertility issue β irregular cycles, anovulation, low progesterone, PCOS, hypothalamic amenorrhea β ends up tracing back to one feedback loop: the HPO axis.
The three players
- H β Hypothalamus. A region of your brain that monitors stress, energy, sleep, and body fat, then sends pulses of GnRH to your pituitary.
- P β Pituitary. A small gland that responds to GnRH by releasing FSH and LH, the two hormones that talk directly to your ovaries.
- O β Ovaries. They respond to FSH and LH by maturing follicles, ovulating, and producing estrogen and progesterone β which feed back to the brain.
Why it can break
The HPO axis is exquisitely sensitive to:
- Chronic under-eating or over-exercising
- Persistent psychological stress
- Disturbed sleep
- Insulin resistance and PCOS-pattern signaling
- Thyroid dysfunction
Any of these can quiet the GnRH pulse β and once GnRH goes quiet, ovulation goes with it.
What you can do this week
- Eat enough. Especially carbohydrates. The hypothalamus reads low energy as "now is not a safe time to reproduce."
- Sleep on a schedule. Even 7 hours, same window each night.
- Track your cycle. Length, ovulation signs, luteal phase. You're looking for patterns, not perfection.
